Graduates of Downers Grove South High School may request immunization records by submitting the completed Release of Immunization Records form.

Please allow 3 to 4 school days for requests to be processed.  Records will be made available for pick-up in the Health Services Office, Room A106.  If records are to be mailed or faxed, please provide the appropriate contact information.
